1. Limit 1 per person and subject to availability.
2. Must be 18 years of age or older to use vouchers (there is no cash value).
3. Free Consultation must be face to face.
4. Free Consultation includes one (1) face to face appointment with Dr. Fisher or one of his medical staff to discuss your weight loss goals and medical history. The scope of this consultation is to determine if you are a candidate for Dr. Fisher’s Medical Weight Loss or Body Shaping Program. The Free Consultation offer does not include a medical examination, which is needed before you can begin one of Dr. Fisher’s Medical Weight Loss or Body Shaping Programs. If after the free consultation you would like to proceed with the examination, the discounted fee for this service is $30. If after the examination you decide to move forward with one of Dr. Fisher’s Medical Weight Loss or Body Shaping Programs, this fee will be waived.
5. Free Consultation offer expires 14 days from the date you sign up for the deal.
6. 24 hour cancellation notice is required if you need to change your appointment time.
